What is IBS?

Irritable bowel syndrome is a functional GI disorder affecting the stomach and intestines. It is generally characterized by diarrhea, constipation, or a mix of bowel habits. IBS can include indigestion, intestinal inflammation, and increased intestinal barrier permeability, as well as general GI pain and discomfort.

The subtypes of IBS are based on stool appearance:

  • IBS with constipation (IBS-C): Stool is hard and lumpy
  • IBS with diarrhea (IBS-D): Stool is thin and watery
  • IBS with mixed bowel habits (IBS-M): Stool is a mix of both hard and watery movements

The subtypes are important because they impact how your IBS is treated. Certain treatments only work for certain types of IBS.

IBS Signs & Symptoms

Any major changes in bowel habits and general gut conditions can signal IBS. Persistent bloating, cramping, diarrhea or constipation, and abdominal pain are all core IBS symptoms, but your symptoms will depend on your type of IBS. Symptoms can either be persistent or they may flare up. Flare-ups could be related to diet and you may find that certain foods trigger symptoms.

Causes of IBS

The cause of irritable bowel syndrome isn’t always clear, but it can be brought on by major changes to the gut microbiome, like a course of antibiotics or food intolerance. It is considered a neurogastrointestinal disorder due to gut-brain interaction. Communication challenges between the gut and brain can cause dysmotility, where the GI muscles have problems contracting and moving through the GI tract, or hypersensitivity, where the nerves in the gut are extra sensitive and cause additional pain and discomfort.

Some people find their symptoms are worsened by foods, such as coffee, spicy foods, alcohol, certain fruits and vegetables (high-FODMAP foods), gluten, or artificial sweeteners.

IBS Health Risks

IBS can lead to increased gut permeability, which means your digestive system is more susceptible to bad bacteria and other potentially harmful invaders. This could lead to further health complications and/or permanent gut damage. However, IBS doesn’t increase risk of more serious gastrointestinal conditions like colitis, colon cancer, or Crohn’s disease. It’s also important to note that IBS is not the same as IBD, inflammatory bowel disease.

IBS Treatment

IBS treatment often takes a multi-faceted approach between dietary changes and medical treatment. RDNs often start with an elimination diet to test how you react to different foods. The low FODMAP diet is one of the most popular and successful dietary interventions for IBS. Your RDN will likely work in partnership with your gastroenterologist and broader care team for a holistic approach.

IBS treatments can include:

  • Dietary changes like higher fiber, limited dairy, limited gas-inducing foods, and the low-FODMAP diet
  • Lifestyle changes like regular exercise, quality sleep, and daily de-stressing techniques
  • Therapeutic interventions like cognitive behavioral therapy and hypnotherapy
  • Medication like antibiotics, antidepressants, and medication specially used for IBS like Bentyl® or Levsin®
