Diagnosis and Treatment Plan

PES: Unintended wt loss related to dx of gastroparesis and self-imposed food restrictions as evidenced by food recall and reported 60 lbs wt loss

Notes: She is at home. Eating the same started to include veggies. She is eating chicken and turkey. She added potatoes and carrots. At times she feels bloated, inflamed. Some days she feels better other she feels worse. She goes w/ her husband to a mall, and they walk for around 30 minutes. She hasn't started to use the journal likely because when she used to write she wrote about her painful past. She is creative likes to build things is the morning, like small house, decorative things in wood. She likes gardening. At lunch time she continues to have some symptoms. She is stressed out because her mother is going to have surgery next week.

She is feeling tired of the symptoms. Te de manzanilla. She found a psychologist in Miami virtual appointment. First appt was on May 29th. She likes her and she is excited about it. Last night sh was crying because it's challenging to accept the gastroparesis.

Nutrition Intervention:

Discussed current behaviors/e going to be hard for her to move forward.

Discussed Vagus Nerve and ways to naturally stimulate it. Yoga, meditation breathing, exercise

Encouraged breathing exercise and meditation. Pt willing to try both

Importance of feeling relaxed prior eating was also discussed and encouraged to walk after her meals to help with digestion

Impressions: She doesn't know who she is without caring for other, giver herself permission, feels guilty, hasn't forgive herself for what happened in the past .

Answered pt's questions and/or concerns.

Verbal/Written education/encouragement given. Gastroparesis Nutrition Therapy

Pt expressed understanding of discussion/goals/recommendations and expect moderate - medium compliance.

Recommended client to schedule an appointment w/ psychologist/therapist.

Monitoring/Evaluation

Action Plan: Dietitian will continue to monitor and remains available to address future nutrition related questions and concerns via chat.

Will monitor journal entries, exercise, food choices. Will assess client progress towards goals and will modify and/or add goals as needed to help client with goal of healthy eating and identify helpful educational materials.  During next session will provide feedback on journal entry. Throughout our session will work with client to increase knowledge about nutrition, mindfulness, movement, mind/body connection, stress managements, self-love/compassion, gastroparesis nutrition therapy

Goals/changes to work on:

1. Walk 2-3 x week for 30 minutes

2. Upload food photos on the Food journal

3. Explore feelings,

4. Add veggies low fiber daily a lunch time (cooked carrot, and monitor tolerance)

5. Fruit daily tried different fruits such as peaches

6. Meditation via app in the morning

7. Make an appointment with psychologist or therapist
