Diagnosis and Treatment Plan

Dietary Intake:

  • Pt states she "slip up" and ate a piece of cake and 20oz of Coke on June 4th at coworker's birthday. Pt states eating that piece of cake makes her carve more sweets.
  • Mom's birthday was the 5th and went to Long Horn steak house in Chandler AZ. Pt ordered steak (8oz sirloin) with lobster and baked potatoes. Pt states she did not get salad because she has been eating salad all week. Pt states the restaurant does offer broccoli and asparagus side orders, but she did not order them.
  • Breakfast this week: Fruit Loops with whole milk, egg white omelets with bologna on toast
  • Work lunch M-F: Salad w/ crouton, hardboiled egg whites, shredded carrots, thousand island dressing. The salad is the size of two palms. She also has marinated chicken thigh with skin on made in the air fryer.
  • Dinner this week: tilapia fish, wild rice, and canned corn with pepper in a can (1 day of the week); pork neck bones with white rice and gravy (not sure what cut of the pork is) (1 day of the week); chicken with dough dumpling (the dumpling has not filling. Pt is unsure what's the dumpling made of since her mom made it) with white rice (1 day of the week); tomato basil chicken with white rice (1 day of the week) from a Facebook recipe.
  • Pt is having fish once a week. Pt intends to try cod fish this upcoming week.
  • Not koolaid or juice this week
  • Snacks: Blend bites 1 step smoothie strawberry flavor- Monday to Friday daily; one Yoplait very vanilla yogurt

Medications and Supplement:

  • Pt states she continues to take Amlodipine, prenatal vitamin, and Cholesterol plus. Pt wants to stop cholesterol plus because it is making her "feel funny". Pt also reports smelly and dark yellow urine.
  • Pt states she BP this week is normal
  • Pt will have her next Lipid panel in ~6 mn.

RD advised pt to stop Cholesterol Plus supplement and discussed symptoms with her cardiologist. RD discussed food sources of sterols and stanols with pt. RD discussed complex CHO vs. simple CHO with pt. RD encouraged pt to continue reduce added sugar intake. RD discussed high niacin content food. RD discussed food sources of probiotics.

Goals for this week:

1. Have bagel with sugar-free PB or try honey nut cheerio for breakfast

2. Substitute white rice with whole grain rice (e.g. wild rice, black rice, brown rice etc..)
